- Local Tour Permit: For local trips of less than 500 miles. File with the Council office two weeks in advance of trip.
- National Tour Permit: For trip of over 500 miles. File with the Council office at least one month before the trip.

- Troop 4 Permission Slip: A general purpose parent permission slip that can be used for any outing. All scouts must have a parent permission slip in order to attend a Troop 4 outing.
- Duty Roster: A patrol duty roster suitable for a weekend basecamp.
- Equipment List: A patrol equipment checklist suitable for a basecamp.
- Menu Planner: Use this to help plan your menu for any outing.
- Shopping List: Once your menu is planned, make sure the grubmaster buys everything by using this handy shopping list.
- Class 1 and 2 Medical Forms: A new Class 1 medical form is required every year; Class 2 are required every three years.
- Class 3 Medical Form: Class 3 medical forms are required every year for Scouts and Scouters over participating in high adventure activities.
- Troop 4 Policy on Conduct: An unfortunate necessity, this document spells out the Troop’s expectations of a Scout’s conduct at a Troop activity. This form needs to be signed by the Scout and a parent each year during registration.
- Consent to Treat Form: This form authorizes our adult leaders in charge of an outing to consent to appropriate medical care for a Scout in the case of an emergency.
- Waiver of Liability: This form limits the liability of Troop 4, its leaders and volunteers from any injuries that may occur during a Troop 4 event.
- Adult Personal Information Form: Fill in this form during registration, or anytime this information changes, so that our Troop database can be kept accurate and current.
- Scout Personal Data Collection Form: Fill in this form during registration, or anytime this information changes, so that our Troop database can be kept accurate and current.
- Resource Survey: This survey informs the Troop leaders about any special skills or resources that you may bring to the Troop. Please fill this out to help strengthen our Troop.
- Climb On Safely: Designed for unit climbing/rappelling, Climb On Safely, is the BSA’s recommended procedure for organizing and managing climbing and rappelling activities at all levels of the Scouting program
- Parental Consent for Climbing / Rappelling: Parental Informed Consent for for participation in Troop 4 climbing and rappelling activities. This form must be signed by both parents / guardians.
- Leader Training Award Record: Adult leaders should use this form to track their accomplishments. Once sufficient requirements have been completed, work with the current Committee Chair to receive your Adult Leader Training knot.
- Merit Badge Counselor Application: Fill in this form, and an Adult Leader Application, to register with the Council as a merit badge counselor. Return this form to the Troop registrar.
- Scoutmaster’s Key Award Record: A Scoutmaster with sufficient tenure and accomplishments is eligible for the Scoutmaster’s Key Award and knot. Use this form to record your accomplishments.
- Backpacking Guide: Full of lots of information, this guide is a valuable resource to both new and experienced backpackers.
- Reimbursement Form: Use this form to request reimbursement for pre-approved Troop expenditures and to report donations.
