BSA Consent and Medical Forms
The following fillable pdf is required annually for all BSA-registered Scouts and Adults. In addition hard copies Are frequently required for specific Scouting event run by various scout BSA Camps and entities*
- BSA Part A (Consent), Part B (Medical History), and Part C (Pre-participation physical).
- Part C is only required if the Scout or Adult intends to participate in outdoor events that last >72 hours. It requires a physician’s examination and signature.
*Unregistered adults do not need to provide medical forms, but parents who participate in Troop activities should Register.
Pro tip – download the pdf and fill in the information with all the information that is unlikely to change in the near future and “save as”, then return to the document and finalize the information that changes and sign and date the document and “save as” again under a new file name.
